Follow these directions to print out a copy of the Bakas Application

The application is in pdf file format.  There are 8 pages in all, and all eight pages can be printed after retrieving the file using an Adobe Acrobat Reader.  If you don't have an Adobe Acrobat Reader you can download a current version (it's free) by clicking on this button:

Once you have the application on your monitor just click on the printer on the tool bar and print the document as you would any other document.  

After printing all 8 pages, fill out the forms as directed and return them to the Bakas Center.  The address is provided on the welcome page of the application.

Note: the application cannot be submitted electronically via this website.
